Admissions Packet (please print off and return only pages with changed information)

• Emergency Contact Information and Protocol
• Student Health Profile
• Over Counter Med Administration
• Prescription/ Nonprescription request
• Waivers (Programs, YMCA, Transportation, Media, etc.)
• Updated Immunization record
- Provide all new Medical forms for any care plan. These need to be redone every year. Click on the forms you need below.

• Individual Care Plan for Students with Type 1 Diabetes
• Food Allergy Action Plan
• Tracheostomy Care Plan

• G-tube/PO/Enteral Feeding Action Plan
• Medications needed at school (this form is found on page 13 of admissions packet)
